Intake: Please complete this intake form at your earliest convenience.

Address: Your session will take place at 1329 Desire St. Please text me at (530)277-9680 when you arrive and I’ll come out to meet you. The studio is through the back entrance at the back of my house, but I’ll meet you in the front yard. I am usually unavailable until the start time of your appointment. If you arrive early, you may text me and wait outside.

Payment: Payment is accepted in cash, check, or zelle/chase quick pay. Please have your payment ready so it doesn’t detract time from our session. You are welcome to pay me from your car or the back yard after the session if paying by Zelle.

Cancellations: I have a strict 48 hour cancellation policy. Should you need to cancel within 48 hours, I will ask my wait list if anyone wants it. If I can’t find someone, you are welcome to fill it with a friend or family member. If we can’t find an alternate, you will be responsible for the total value of your session. I accept penalty-free cancellations due to illness, emergency, or weather that causes public spaces to close. These must be reported at your first knowledge of them for them to waive the cancellation fee.
